Abstract
Zusammenfassung Eine Vielzahl an Möglichkeiten bez. Systeme und Konfiguration für extrakorporale Gasaustauschverfahren steht zur Verfügung. Genaue Planung und Evaluation der individuellen therapeutischen Bedürfnisse für eine maßgeschneiderte Therapie sind zu fordern. Die Indikationsschwelle für eine Therapie mittels extrakorporaler Membranoxygenierung (ECMO) als Lungenersatz sinkt zunehmend. Patienten mit schwerem Acute Respiratory Distress Syndrome (ARDS) ohne Besserung auf optimierte Beatmung und supportive Therapie sind potenzielle ECMO-Kandidaten. Kontraindikationen sind relativ und müssen nach Abwägung des individuellen Nutzen-Risiko-Verhältnisses beachtet werden. Ähnliches gilt für die Entscheidung zum Abbruch einer ECMO-Therapie, für die gute Evidenz fehlt.Keywords
