Abstract
Our patient is an 8-year-old boy who has had episodes of hemiparesis since aged 3 years. Episodes begin with a “particular whimper” during sleep, and within moments he develops hemiparesis. His mother describes stiff extended limbs with dystonic posturing of the hands or feet. The patient’s upper extremities are involved more severely than the lower extremities.
