The evaluation of the relationships between physical factors and effects of exercise intervention on physical functions in community-dwelling older people

Abstract
目的: 本研究は, 地域在住高齢者の介入前の身体機能レベルと運動介入による身体機能改善効果との関係を明らかにすることを目的とした. 方法: 対象は東京都内の7つの自治体の地域在住高齢者276名 (平均年齢75.3±6.5歳) であった. 個別評価に基づいて高負荷筋力増強トレーニングとバランストレーニング等を組み合わせた包括的な運動トレーニングを3ヵ月間行った. 運動介入の前後に最大歩行速度, Timed Up and Go, 開眼・閉眼片足立ち時間, ファンクショナルリーチ, 筋力, 長座位体前屈などの身体機能測定を行い, 各体力要素の改善効果と初期の身体機能レベルとの関係を検討した. 結果: 対象者の運動介入前の平均最大歩行速度は85.8±30.6m/分と虚弱な対象であったが, トレーニングの脱落率は8.0%と低値であった. トレーニング後, 閉眼片足立ちを除き, すべての身体機能において有意な改善を認めた (P<.01). 最大歩行速度の変化量以外, 身体機能の変化量・変化率は, 初期の身体機能レベルと負の相関を示した(|r|=.20~.59, P<.01). また, 重回帰分析の結果, 各身体機能の変化量を説明する変数として複数の身体機能要素が抽出された. 結論: 虚弱高齢者を含んだ対象への運動介入の結果, 身体機能レベルが低い者ほど, 身体機能改善効果が高いことが示された. 適切な対象を選択することがトレーニングの効果を高める重要な点であることが示唆される. トレーニングの対象をより明確にして介入を加える, いわゆるハイリスクアプローチが有効であると考えることができる.This publication has 15 references indexed in Scilit:
