Die Bedeutung der Vererbung bei Kardiomyopathien

Abstract
Inheritance plays an important role in the cardiomyopathies (i.e. dysfunction of the cardiac muscle of unknown origin). This especially has become obvious for the entity asymmetric septal hypertrophy of autosomal dominant inheritance (ASH), a cardiomyopaty with and without obstruction. But this is not the only type of a cardiomyopathy of dominant transmittance as was shown by the demonstration of a kindred, in which echocardiographically no hypertrophy of the septum could be found. For it was not possible to find an uniformity for all the affected members of the kindred, for example a congestive course of the disease. Therefore it is suggested to name the disease without any prejudice “cardiomyopathy without asymmetric septal hypertrophy of dominant inheritance” to distinguish it from the “cardiomyopathy with asymetric septal hypertrophy of dominant inheritance”. There also may occur a cardiomyopathy of autosomal recessive inheritance. This form could represent a large part of the “sporadic” cardiomyopathies and should challenge to search intensively for enzyme defects as a cause of the disease. Die Vererbung spielt bei den Kardiomyopathien (Dysfunktion des Herzmuskels unbekannter Ursache) eine wesentliche Rolle. Dies ist besonders deutlich geworden bei der Entitität „Autosomal dominant vererbte asymmetrische Septumhypertrophie (ASH)“ als Kardiomyopathie mit und ohne Obstruktion. Sie ist aber nicht die alleinige Form einer dominant vererbten Kardiomyopathie, wie anhand einer eigenen Sippenuntersuchung gezeigt wird, bei der echokardiographisch keine asymmetrische Septumhypertrophie nachgewiesen werden konnte. Da kein Befund (z.B. kongestiver Verlauf) erhoben werden konnte, der diese Kardiomyopathie bei allen erkrankten Sippenangehörigen charakterisiert hätte, wird vorgeschlagen, das Krankheitsbild unvoreingenommen als „Dominant vererbte Kardiomyopathie ohne asymmetrische Septumhypertrophie“ zu bezeichnen und sie so von der oben erwähnten Entität „Autosomal dominant vererbte Kardiomyopathie mit asymmetrischer Septumhypertrophie“ abzugrenzen. Darüberhinaus wird darauf hingewiesen, daß es auch autosomal recessiv vererbte Kardiomyopathien gibt. Sie könnten einen wesentlichen Anteil der „sporadischen“ Kardiomyopathien ausmachen und fordern zu intensiver Suche nach Enzymdefekten als Ursache auf.Keywords
