Selective intellectual deficits in children with temporal lobe or centrencephalic epilepsy

Abstract
Specific patterns of intellectual impairment were observed in children with unilateral temporal lobe or centrencephalic epilepsy. The left temporal patients showed learning and memory deficits on verbal tasks while their performance on nonverbal tasks was unaffected. The converse was true for children with right temporal involvement. In contrast to the two temporal groups, the centrencephalic patients were without significant memory defect; instead, they showed lower scores on a test of sustained attention. The results are similar to those obtained in studies of adults with comparable cerebral dysfunction. The findings suggest that cerebral differentiation of intellectual processes is established in childhood. Des types spécifiques de déficit intellectuel ont été observés chez les enfants avec épilepsie d'origine temporale unilatérale ou d'origine centrencéphalique. Chez les malades avec lésion temporale gauche, il existait des déficits d'apprentissage et de mémorisation dans des épreuves verbales, tandis que leurs performances sur des tâches non verbales n'étaient pas affectées. L'inverse était rencontré chez les enfants avec atteinte du lobe temporal droit. Constrastant avec ces deux groupes temporeux, les sujets avec épilepsie centrencéphalique ne montraient pas de déficit significatif de la mémoire, mais ils avaient des scores très bas dans un test de maintien de l'attention. Ces résultats sont similaires à ceux qui ont été obtenus chez des adultes présentant des dysfonctions cérébrales comparables. Ces constatations suggèrent qu'une différenciation cérébrale des processus intellectuels s'établit dès l'enfance. Spezifische Muster intellektueller Beeinträchtigung wurden bei Kindern mit einseitiger Temporallappen- oder zentrenzephaler Epilepsie beobachtet. Die Patienten mit Schädigung des linken Temporallappens zeigten bei verbalen Aufgaben Lern- und Gedächtnisdefizite, während sie nichtverbale Aufgaben unverändert ausführten. Umgekehrt war es bei den Kindern mit rechtstemporaler Schädigung. Im Gegensatz zu den beiden temporalen Gruppen waren die Patienten mit zentrenzephaler Epilepsie ohne signifikanten Gedächtnisdefekt; statt dessen zeigten sie schlechtere Ergebnisse bei einer Prüfung der Aufmerksamkeitsausdauer. Die Ergebnisse ähneln denjenigen, die man bei Untersuchungen von Erwachsenen mit vergleichbarer zerebraler Funktionsstörung erhielt. Die Beobachtungen legen es nahe, daβ eine zerebrale Differenzierung intellektueller Prozesse in der Kindheit erfolgt.Keywords
