A Retrospective Review of Poor-risk Primary Lung Cancer Patients Who Underwent Wedge Resection

Abstract
目的.原発性肺癌の縮小手術として楔状切除(以下WR)はしばしば用いられているが,その適応はなお不明確である.消極的縮小手術としてのWRの治療成績を検討した.方法.2007年4月より2020年3月までに原発性肺癌に対して消極的縮小手術としてWRが施行された93例を対象とし,その外科治療成績を検討した.結果.完全切除は84例(90.3%)に施行し得た.病期別にみると,消極的縮小手術群の3および5年生存率はIA期でそれぞれ74.7%と67.3%,IB期で66.3%と41.4%,II期で48.0%と24.0%であった.予後因子解析では,腫瘍径の予後因子としての意義は確認できず,性別,組織型,臓側胸膜浸潤が有意な予後因子となった.術後再発は29例(34.5%)に認め,その内訳は遠隔再発14例,局所再発13例(うち断端再発8例),両者複合再発2例であった.術後合併症は9例(9.7%)に認めたが,重篤なものはなく,積極的WRの合併症発生率との間に有意差はなかった.結論.I期肺癌において,消極的縮小手術WRは認容性の高い術式と思われる.This publication has 19 references indexed in Scilit:
