Endoprothetik an der Hand und am Handgelenk

Abstract
Zusammenfassung. Bereits Ende des 19. Jahrhunderts wurden erste endoprothetische Eingriffe durchgeführt, jedoch begannen sich Mediziner erst Mitte des 20. Jahrhunderts intensiver mit dem Gelenksersatz im Bereich der Hand zu befassen. Heute gibt es für fast jedes Gelenk der Hand und des Handgelenks eine Auswahl an Prothesen. Im Bereich der Fingergrund- und Mittelgelenke konkurrieren neuartige Prothesen mit dem seit Jahrzehnten etablierten Silikonspacer. Auch am Daumensattelgelenk konnten Prothesen aufgrund hoher Lockerungsraten in der Vergangenheit die bewährte Trapezektomie (mit Suspensions- / Interpositions-Arthroplastik) nicht ablösen. Am Handgelenk können bezüglich Schmerzlinderung, Funktion und Greifkraft gute Resultate erreicht werden, jedoch besteht eine höhere Komplikationsrate im Vergleich zur Gelenksversteifung. Die Werte verbessern sich zwar mit den neuen Prothesenmodellen, jedoch fehlen Langzeitstudien, vor allem für jüngere Patienten mit hohem funktionellem Anspruch.Keywords
