Abstract
医源性胆管损伤在临床上屡见不鲜,且胆管损伤后果严重,是肝胆外科医生面临的一个较为棘手焦点问题。胆管损伤后如果处理不当,可能导致胆管狭窄、反复发作的胆管炎、胆汁淤积性肝硬化甚至肝衰竭而死亡,对胆管损伤的诊断、分型及处理方式,直接影响病人预后,因此临床医师需要对胆管损伤有充分的认识,术中要做到仔细辨认局部解剖关系,谨慎操作,一旦发生医源性胆管损伤,要准确把握手术时机,选择合理的手术方式,改善病人的长期预后。 Iatrogenic bile duct injury (IBDI) is common in clinical practice, and the consequences of bile duct injury are serious. It is a thorny focus for liver and bile surgeons. If the bile duct injury is not handled properly, it may lead to the death of bile duct stenosis, recurrent cholangitis, cholestasis cirrhosis, and even liver failure. The diagnosis, classification, and treatment of bile duct injury directly affect the patient’s prognosis. Therefore, clinicians need to have a full understanding of bile duct injuries. During surgery, they must carefully identify local anatomical relationships and operate cautiously. Once IBDI occurs, they must accurately grasp the surgical timing, select reasonable surgical methods, and improve the long-term prognosis of the patient.
